Stages of Pressure Ulcer

STAGE	DESCRIPTION
Stage I	The skin is intact with non-blanchable redness over a localized area
Stage II	Involves partial-thickness loss of the dermis
Stage III	Involves full thickness loss, but bone, tendon, and muscle are not exposed
Stage IV	Involves full-thickness loss with exposed  bone, tendon and muscle
